Shouldice Hospital, Ltd. Case

Starting as a small hospital, the Shouldice Hospital has had a rather long history since the end of the Second World War. And At the time of his death in 1965, the founder Dr. Shouldice’s longtime associate, Dr. Nicholas Obney was named surgeon-in-chief and chairman of the board of Shouldice Hospital Limited and under his leadership, the volume of activity continued to increase, reaching a total of 6,850 operations in the 1982 calendar year. And primaries represented approximately 82% of all hernias operations performed at Shouldice in 1982.It has been very successful. And now it tries to meet people’s needs and make this as good a place to work as possible. It has been a comfortable place for patients to stay. During their stay, patients were encouraged to take advantage of the opportunity to explore the premises and make new friends. Also, it has been a nice place for doctors who teach each other, encourage a group effort, have regular working hours and own desirable salary. Furthermore, the facility and administration here is very humanized for both patients and staff.As a result, patients are attracted to the hospital by its reasonable rates. Shouldice Hospital has specialized in hernia surgery (the Shouldice method) and is able to provide patients with low risk, low recurrence, and peace of mind. Shouldice achieves outstanding results as a low price and at high profit because everything done by the hospital is designed to maximize the difference between perceived quality and the value of the service provided to the patients on one hand and the cost of providing the service on the other. They do this by carefully screening patients.Patients are active participants in the service delivery process as presented with the utensils to prep themselves for surgery. The avoidance of general anesthetics allows a wide range of involvement of patients. The staff is freed from much of the disagreeable workload due to part-time positions. The cost of housek eeping, laundry, and food is also minimized. The clinic structures hospital jobs that involve more counseling and positive interaction with the patients. Shouldice Hospital is different from other hospitals in that Patients do not feel that they are staying in a hospital.It is Shouldice’s intentions to make its patients feel as if they are taking a vacation. When patients arrive to the clinic, typically they join up with 30 to 34 other patients and their friends and families in the waiting room. Patients with similar jobs, backgrounds, or interests are assigned to the same room to help the patients establish relationships and become more comfortable. The facility is also designed so that patients have to interact and walk around. There are no televisions are telephones in the patients’ rooms.This gives the patients the opportunity to tour the facility, visit each other’s rooms, lounging in the Florida room, and making use of the light recreational facilities. Sh ouldice Hospital uses a focus strategy. The Hospital’s market focus is on a narrow segment of potential patients who have a hernia and the patients are predominantly male, older in age, and essentially in good health. The hospital has knowledge of the large target market, as there were 600,000 such operations in the United States in 1979. Shouldice uses an internal market strategy.Shouldice’s doctors are dedicated to quality of the Shouldice method. The doctors are after several years at Shouldice must decide if this is something they want to do for the rest of their lives because eventually they will lose touch with other medical disciplines. Few elect to leave. Nurses at the facility are people oriented and work to assist patients as well as they can. The Staff is flexible and team oriented. Shouldice implements a sharing policy where the employees are paid a part of the profit in addition to their fixed salaries.This sharing is done based on their contribution towar ds the success of the firm. This entices them to work hard and put in more efforts. From the management point of view, they have given their staff a lesser work load and weekends off. This makes it able for the staff to spend more time with their families and keeps them content with their jobs. After discussion, our group has four suggestions for Shouldice Hospital in terms of their problem. First, author mentions that there are some people realize that they don’t have hernias when they talk to doctor in hospital.Therefore, hospital can increase its capacity by reduce this kind of error. Our group suggests that hospital should improve its questionnaire to make sure consumer can totally understand the questions. If there are some uncertainties in the questionnaire, nurses or doctors should call individual to make sure he/she has certain external types of abdominal hernias. Secondly, we think that Shouldice Hospital should not operate on Saturday. Even through Saturday operatio n can increase capacity by 20%, it is hard to control the operation quality. As O’Dell said† On the other hand, I'm oncerned about this talk of Saturday operations. We are already getting good utilization of this facility. And if we expand further, it will be very difficult to maintain the same kind of working relationships and attitudes. Already there are rumors floating around among the staff about it. And the staff is not pleased. † Right now, Shouldice hospital is much more efficient and effective than other hospital. We believe they make excellent utilization of their resource. Therefore, Saturday operation may cause some overload on hospital’s capacity which will jeopardize the current process flow.Thirdly, we believe that offers similar opportunities such as eye surgery is not a good idea. The reason is that the business bottleneck of Shouldice Hospital is the capacity of process rather than the demand in market. So, they should focus on how to impro ve the capacity to service more patients. Moreover, offering other surgery may harm their professional image in hernias area. Hence, our group suggests that they just stay on the hernias field and try to extent capacity. Fourth, we strongly suggest that Shouldice Hospital should build another hospital in the United States.Nowadays, 42% of their patients are come from the United States. Round-trip fares for travel to Toronto from various major cities in the North American continent ranged from roughly $200 to $600, as compare that the total operation fee including hospital is just around $1000. So the travel cost becomes a big financial burden for patients. Hence, open a new hospital in American will dramatically reduce the travel cost for customers and expand business. There are two big issue concerns with the new hospital, where to open and how to operate.Our group proposes that Shouldice Hospital should conduct a marketing research to find out where their current American patient come from and the transportation for some big city in United States to decide where to invest new hospital. About the operation of new hospital, quality control is the most important issue to maintain our brand reputation. They should relocate some experienced doctor from Toronto to American to train and direct the local doctor. Moreover, they should utilize the successful process flow into the new hospital and make some change by considering the difference culture and condition.Finally, Shouldice Hospital should be marketing their service, while the main strategy of marketing is to clarify where is hospital and what kind of operation is truly theirs. Because there are many individuals or institution on the market want to attract customers by using our brand image. But, the quality of their service is not very good. This phenomenon will jeopardize our brand image and destroy our reputation. It is the responsibility of hospital to help people distinguish the fake one. Our group belie ves that some marketing activity will well-build hospital’s brand image and help them success in the long-term.

Legal and Ethical Implications Essay

In the classroom there is responsibility and duties for the teachers, students, as well as the parents. These responsibilities help the classroom to run smoothly and they help to build a well-managed classroom. Of those responsibilities teachers, students and parents need to understand legal and ethical issues in classroom management. This paper will discuss two articles and address the legal and ethical issues for classroom management. It will address how the articles relate to the rights and responsibilities of students, teachers, as well as the parents. Lastly, I will discuss how each article will make a difference in the way I will manage my classroom in the future. Article One: Untangling Legal Issues that Affect Teachers and Student Teachers This article discussed legal issues and focused on child abuse, safety in the art room, freedom of expression and religion, and special education and how these topics affects art teachers as well as student teachers. From the article I lear ned that the Tenth, First, and Fourteenth Amendments hold significance for educational law in the United States. Next, from the article I read that it is required by law that a teacher reports child abuse. The article defined child abuse as a non-accidental, patterns of injuries, sexual abuse, neglecting treatment, physical or mental injuries, or mistreatment of a child under the age of eighteen (Untangling Legal Issues that Affect Teachers and Student Teachers, 2009, page 48). Child abuse is a federal crime and teachers could be held responsible if they don’t report it. If it is not reported a teacher can receive a misdemeanor and the penalty could range from spending time in jail to being fined. Safety in the art room is another topic discussed in this article. From the article I learned that a teacher could be held liable for injuries if the injuries are found to be negligent. An example of a negligent injury is teacher not demonstrating to students the proper care and handling of materials and tools. Art teachers should not assume that materials are safe and they should send out permission forms that let parents know what material students will be working with, have parents lists their contact information, and any know allergies the child may have. The article states that the art room should be set up properly, hazardous materials should be labeled, and teachers should lock up hazardous materials. This article discusses how children with exceptional needs cannot be excluded public education. Also, I read in the article about freedom of religion and how teachers cannot preach their beliefs in school nor have religious conversations with student and I read how students have freedom of expression but it is the art teacher responsibility to make sure the students art work is displayed in a way that is appropriate. Then the article goes into detail to discuss the liability of student teachers. From the article I read that student teachers are responsible for the same duties as full time teachers and the liability is the same as also. After reading this article it has impacted how I will manage my class. It made me realize how important it is to get to know all of my student and their parents, how to pay careful attention to my students, how to set my classroom up appropriately, and make sure I demonstrate how to use materials appropriately. It definitely made me realize the importance of freedom of expression and religion. I will respect all my students and never try to force my religion on them. Also, I will report child abuse if I see it. I didn’t realize how important all these things were but this article made me aware and my outlook on classroom management is differently. When managing my classroom I will make sure it incorporates all these things like freedom of expression, religion, and I will make sure the classroom is set up in a way that I am able to monitor my students for things like child abuse and to make sure they use certain materials appropriately. Article Two: Using Time-Out Effectively in the Classroom In this article it discusses how to use time out effectively and how it could lead to legal problems if used ineffectively. From the article, time out is defined as a procedure or form of punishment where students are denied access to all opportunities for reinforcement and it is used when inappropriate behavior is displayed (Using Time Out Effectively in the Classroom, 2007, page 60). Time-out has been used by many teachers to address inappropriate behaviors in the classroom. There are four different types of time-out and they are: inclusion time-out, exclusion time-out, seclusion time-out, and restrained time-out (Using Time-Out Effectively in the Classroom, 2007). Inclusion time-out involves removing the reinforcement from the student. Some variations that involved in this type of time-out are ignoring and removal of materials. Exclusion time-out is when the student is removed from the enhancing setting that caused the behavior. Exclusion time- out requires a student to be removed from instructional activities. Restrained time-out which is the most preventive form of time-out requires both a restraint and restriction. Restrained time-out is when the teacher places the student into a time-out position and maintains the student and position using physical restraint (Using Time Out Effectively in the Classroom, 2007, page 62). Isolation time- out better known as Seclusion time-out involves the teacher removing the student from the classroom and places them in a room or assigned area where he or she is restricted from leaving until they serve their time in this area. From the article I read that time-out becomes ineffective when typical pitfalls include classroom environments that are insufficiently reinforcing to the students or when time-out procedures lose their punishing qualities and take on reinforcing qualities (Using Time-Out Effectively in the Classroom, 2007, page 62). Also, I read over some recommendations on how to make the classroom reinforcing, use effective teaching strategies, how to come up with a time-out plan, and how to develop a hierarchical behavior management plan. I learned that we as teachers should be trained in methods of controlling behaviors. I learned some ways time-out is used inappropriately. For example, a teacher used it for a mild behavior. By doing this it could result to legal problems. After reading this article it has definitely changed my view on how I will manage my class in the future. When I manage my class, I plan to apply time-out effectively and use it in a way that is appropriate and will reduce the misbehavior within the student. Next, I will make sure I develop a hierarchical behavior management plan as well as a time-out plan so that the behavior and time-out is managed properly. Next, I will document when I use time- out which includes important information like the student name, date, behavior, duration, etc. By doing this, this can assist staff in determining if time-out was effective an d this could reduce chances of legal problems. I will make sure I establish a time out policy and I will go over this policy with my students on the first day of school and send a copy home to the parents to make sure they understand the time-out policy. Conclusion There are many responsibilities for parents, teachers, and students when it comes to managing a classroom. Teachers need to make sure they take the proper precautions and follow the state laws and schools policies when managing a classroom, and students need to make sure they understand the rules, policies, and procedures in the classroom. Also, parents need to be notified of these rules, policies, and procedures. The Drunkard by Frank O’Connor

â€Å"The Drunkard† is, at first glance, a comedy about youth and unintended drunkenness, and a grateful mother. It is also, however, a glimpse into the personalities of the members of a dysfunctional family: egotistical father, demeaned mother and ignored and/or abused child. Think about the story and choose a focus of your essay. It could be humor, family dynamics or the individual characteristics of the father or son. The Use of Irony and Humor of â€Å"The Drunkard â€Å"In the story, â€Å"The Drunkard† the author Frank O’Connor uses a point of view to primarily reveal humor and irony. In the story, O’Connor uses first person point of view. First person point of view is told through the eyes of the main character in this story. The main character is named Larry. Larry is a young boy who has to go along with his father one day to a funeral.Larry’s father Mick is the person referred to as the â€Å"drunkard†. Larry seems to know what will happen when he goes with his father but is hoping that his father might consider his presence and not drink. Since the story is told through little Larry’s eyes and thoughts the reader mainly focuses on how Larry feels about his dad being a drinker. The Irony and humor that is found in human nature is revealed through Larry, first person point of view, and what happens on his outing with his father Mick.Irony, the incongruities between the expected and actual results of events and humor, the quality of being laughably ludicrous are interweaved in this story. In the short story, â€Å"The Drunkard† Frank O’Connor uses first person point of view to reveal the humor and irony that is created in this amusing story. Humor is seen many times in the story after Larry and his fath er Mick reach the bar following the funeral. Larry is thirsty and takes a drink of his father’s beer. Larry finishes his father’s drink and becomes drunk afterwards.While this is occurring his father is talking away with a man named Peter Crowley who is also a drinker. When Mick realizes Larry is drunk he knows he must take him home immediately. This scene is described as, â€Å"They all stopped gabbling to gape at the strange spectacle of two sober men, middle aged men bringing home a drunken small boy with a cut over his eye†(301). This could not be better told than through the eyes of Larry who at the time is observing all he sees happening around him. Two usually drunk men carrying home but a young boy who is not sobers enough to walk.This humorous scene described by the main character reveals more enjoyment because little Larry is the one who is drunk. It is also a bit ironic that the two grown men are carrying a drunken young boy home and it is not the oth er way around. Larry is watching all the people around him and knows how ridiculous he must look in between the arms of his father adepter Crowley. The humor is revealed in an entertaining way from the first person point of view because of the situation the main character is in and how he is describing it.Irony seems to occur in a few instances since Larry is setting up the reader with certain expectations. Larry tells us how his father is and knows exactly what will happen after the funeral. His father will wind up in a bar drunk like he had been described doing since his best friend passed away. Larry’s first conclusion about the circumstances that are likely to occur is, â€Å"I know I might have to bring him home, blink drunk, down Blarney Lane, with all the old women at their doors, saying: ‘Mick Delaney is on it again’† (302).This is Larry’s prediction to how the day would end up. To the reader’s hilarious surprise, this is not what hap pens. Larry’s thirst at the bar gets him in to an unusual situation and bewilderment from the liquor he has drunk. Larry as he realizes he is drunk says to himself, â€Å"But, drunk and all as I was†¦Ã¢â‚¬  (301). both the Larry and the reader are surprised by this ironic event. Larry’s prediction is reversed. He no longer has to take his drunken father home down Blarney Lane. Now Larry’s father has to take Larry home blind drunk.The twist of events that Larry the main character is going through mainly reveals the irony also in a more humorous way. Humor through the eyes and words of Larry, the main character, provide the reader with more enjoyment as his day continues. As the men carry Larry by the arms he knows he is going to stroll down Blarney Lane drunk. So as Larry is proceeding down the lane he cries out to the women laughing, â€Å"I’ll make ye laugh at the other side of year faces if ye don’t let me pass†¦Go away ye bloody bit ches†¦Take care or I’ll come back and show ye!† (302).This scene described by Larry is very comical. He knows he father usually passes down this same lane drunk but does not get to realize what it is like until now. It’s especially worse for Larry because he is not a grown man yet who is allowed to drink. It is ironic also because Larry never thought he’d be the one walking down this lane drunk after the funeral. The humor being revealed is more amusing through the words of Larry who is telling the story from the first person point of view.Using the first person point of view is more appropriate in order to reveal humor and irony as the main elements in this story. The main character Larry who is telling this story adds a more entertaining view. He gives the story irony from his own words and predictions. He also gives the story more humor because of his actions. Despite what he thought the day would be like with his father after the funeral he ends u p being mistaken in an amusing way. The story told from the first person point of view reveals a more enjoyable story, regardless of whether the story’s incidents were actually true or not.

In their 1996 study, Vrij and Semin examined this question by recruiting not only college students, but also subjects from a variety of fields thought to be trained and experienced in lie detection (police patrol officers, prison guards, customs officials, etc.) as well as those constantly exposed to an environment based on lies and deception (prisoners). It was their hypothesis that law enforcement officials would be better at lie detection than "normal people" such as college students and that the criminals would have superior abilities to those of law enforcement. All of the subjects reported that they considered themselves highly experienced at detecting deception (Vrij & Semin, 1996). Vrij and Semin administered their study through a series of questionnaires administered at the subjects' places or work or incarceration. These questionnaires contained questions ranging over 16 nonverbal cues commonly associated with deception and concluded with the subjects rating themselves on their ability to detect deception in others on a 7-point scale. The results were compiled by group (college student, law enforcement, or criminal) and then analysed for accuracy. As predicted, the criminals exhibited the greatest level of accuracy in correctly identifying nonverbal cues of deception. It was also found that there was not a significant difference between the accuracy rates of law enforcement and college students, indicating that those considering themselves professional lie detectors fell victim to the same misconceptions on nonverbal cues as a lay person (Vrij & Semin, 1996). While Vrij and Semin's study does provide useful data that professional lie detectors have no superior knowledge of deceptive nonverbal cues than the rest of us, it does lack in that none of the subjects were actually tested in their ability to detect deception. This study will address that lack by actually testing whether or not subjects can detect deception through nonverbal cues, particularly by focusing on voice pitch. It is predicted that such attention to nonverbal cues will detect deception at a higher rate than chance and that those observers relying on voice pitch as a deceptive cue will be more accurate than observers relying on other nonverbal cues. Methods A pool of 761 subjects was recruited consisting of 454 females and 307 males who were informed that they would be participating in an undergraduate psychology tutorial study. These subjects ranged in age from 17 to 56 years with a mean age of 19.77 years and a standard deviation of 2.58 years. For this study, each subject was instructed to tell a story to another subject.
